Несмотря на стабильно высокий охват детей вакцинацией против коклюша, последнее десятилетие в России характеризуется устойчивой тенденцией к росту заболеваемости этой инфекцией. Болеют как непривитые, так и привитые дети. Рост иммунной прослойки сопровождается увеличением доли привитых среди заболевших. Новизна работы заключается в обосновании концепции неполной защиты при вакцинации при коклюше, подразумевающего неполную защиту от инфицирования при сохранении защиты от тяжелых форм и летальных исходов. В рамках этой концепции впервые проанализированы особенности клинической картины как проявления данного заболевания, ограниченного остаточным иммунитетом. С эпидемиологических позиций доказано, что когорты привитых лиц со слабым иммунитетом могут проявляться. С помощью непараметрического корреляционного анализа (ρ Спирмена) выявлена умеренная обратная связь между уровнем поствакцинальных антител и длительностью заболевания у привитых (ρ = -0.42, p=0.03), что подтверждает влияние остаточного иммунитета на клиническую картину.</p></abstract><abstract xml:lang="en" abstract-type="summary"><p>Whooping cough remains one of the most pressing health issues. Despite the consistently high vaccination coverage of children against whooping cough, the last decade in Russia has been characterized by a steady increase in the incidence of this infection. Both unvaccinated and vaccinated children are affected. The growth of the immune population is accompanied by an increase in the proportion of vaccinated individuals among those who become ill. The novelty of this work lies in the justification of the concept of «ephemeral immunity» in the case of whooping cough, which implies incomplete protection against infection while maintaining protection against severe forms and fatal outcomes. Within the framework of this concept, the features of the clinical picture as manifestations of «partial pathogenesis» limited by residual immunity were analyzed for the first time.
